Job summary

Providence Willamette Falls Medical Center is seeking a Licensed Clinical Social Worker (LCSW) to provide biopsychosocial assessments and evidence-informed care plans. You will collaborate with the Inter/Multi-Disciplinary Team (IDT) to support inpatients and outpatients across the care continuum.

The role emphasizes patient self-determination, equity in access to care, and adherence to NASW ethics and mission.

Qualifications

• Master’s Degree in Social Work from a CSWE-accredited school of social work.
• Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License upon hire.
• License in states where care is delivered and 2 years of relevant social work experience.

Responsibilities

• Independently provides biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions for patients and support systems.
• Collaborates with patients and the IDT to develop evidence-informed, person-centered care plans.
• Adheres to NASW ethics, SOP, and Providence mission while reducing health disparities.
• Works with inpatients and outpatients across care continuum, respecting patient self-determination.
