Job summary

Affinity Health Management is seeking an On-Call Hospice RN to respond to patient census and family inquiries after hours. Responsibilities include urgent clinical and psychosocial interventions, pain and symptom management, and collaboration with the hospice team to maximize outcomes.

The role requires a licensed RN in NJ, strong communication, and the ability to work independently in crisis situations, with driving responsibilities to visit home-based patients.

Qualifications

• Graduate of an accredited school of nursing.
• Current unencumbered RN license to practice in NJ.
• Preferably 2 years of hospice/homecare or related nursing experience.
• Must have a valid driver’s license and insured vehicle.
• Willing to work after-hours shifts.

Responsibilities

• Available by telephone and email during assigned shifts.
• Perform urgent physical and psychosocial assessments after hours.
• Provide safe environment and professional nursing care using the nursing process.
• Pain and symptom management with skilled nursing care.
• Guide patients and families on diagnosis, medications, treatments and progress.
• Document care thoroughly and timely in the patient chart.
• Communicate with physicians and IDG; obtain orders as needed.
• Collaborate with hospital and community staff to coordinate care after hours.
• Assist with after-hours admissions and mandatory meetings.
• Engage in ongoing professional development and training.
