Job summary

Affinity Health Management in New Jersey is seeking an On-Call Hospice RN to respond to patient census, families, and referrals after hours (nights, weekends, holidays). Qualifications

• RN license in New Jersey (active).
• Graduate of an accredited school of nursing.
• Hospice/homecare experience preferred.
• Ability to document in a timely manner and comply with HIPAA.

Responsibilities

• Respond to census and after-hours inquiries by telephone and email.
• Conduct urgent physical and psychosocial assessments after hours.
• Provide pain and symptom management and a safe care environment.
• Document care thoroughly and timely; record changes.
• Collaborate with physicians and the IDG to coordinate care.
• Arrange DME and medications after-hours as needed.
• Participate in mandatory meetings and continuing education.
