Job summary

Affinity Health Management is seeking an On-Call Hospice RN to respond to patient census and family inquiries outside of standard hours. The role provides urgent clinical and psychosocial interventions, with emphasis on pain and symptom management and collaboration with the hospice team to maximize patient outcomes.

The position requires independent assessment in crisis situations, coordination with the Hospice Medical Director, and after-hours admissions.

Qualifications

• Graduate of an accredited school of nursing.
• Maintains current unencumbered RN license to practice in Florida.
• Preferably has two years of hospice, homecare, hospital medical-surgical or oncology nursing.
• Must be a licensed driver with insured vehicle and reliable transportation.
• Ability to document thoroughly and work within state/federal regulations.

Responsibilities

• Responds to Hospice patient census and family inquiries after hours.
• Provides urgent clinical and psychosocial interventions and pain/symptom management.
• Collabrates with the Hospice Medical Director and administrator on call as needed.
• Documents care thoroughly and timely; maintains relevant clinical records.
• Assists with after-hours admissions and coordinates with IDG and vendors for supplies.
• Maintains safety, confidentiality and professional standards during crises.
