Job summary

Affinity Hospice Management LLC in New Jersey is seeking an On-Call Hospice RN to respond to census, referrals and family inquiries after hours, including nights, weekends and holidays.

The role provides urgent clinical and psychosocial interventions, pain and symptom management, and coordination with the hospice team to maximize patient outcomes, with independent assessment in crisis situations.

Qualifications

• RN licensed and in good standing in NJ.
• Must understand hospice philosophy of care.
• Demonstrates ability to work independently and in crisis situations.

Responsibilities

• Available by telephone and email during assigned shifts.
• Conduct urgent physical and psychosocial assessments after hours.
• Provide and maintain a safe home environment.
• Deliver skilled nursing care using the nursing process.
• Focus on pain control and symptom management.
• Guide patient and family on diagnosis, medications, treatments and progress.
• Document care thoroughly and timely.
• Coordinate with physicians and the IDG for continuity of care.
• Collaborate with vendors for DME and supplies after hours.
• Participate in mandatory meetings and continuing education.
