Job summary

Affinity Health Management is seeking a dedicated Hospice On Call Registered Nurse to respond to after-hours census and family inquiries. The RN will provide urgent clinical and psychosocial interventions, manage pain and symptoms, and collaborate with the hospice team to maximize patient outcomes.

The role requires a Florida RN license, at least two years in hospice or related fields, and the ability to travel to patient homes.

Qualifications

• Florida RN license in good standing.
• Two years hospice/homecare or related nursing experience.
• Ability to travel to patient homes and drive legally.
• Able to work after-hours on-call shifts and weekend work.
• Strong communication and documentation skills.

Responsibilities

• Respond to Hospice patient census and family needs during after-hours.
• Provide urgent clinical and psychosocial interventions to patients and families.
• Collaborate with providers within and outside the hospice team.
• Document care thoroughly and timely using standard nursing processes.
• Coordinate with IDG and vendors for DME, supplies, and meds after hours.
• Assist with admissions and participate in required meetings.
