Job summary

New Jersey Hospice Care is seeking an experienced RN On-Call Nurse to provide compassionate hospice care across North New Jersey. The role covers on-call visits, symptom management, and coordination with the interdisciplinary team.

You will assess patients, administer medications, document in accordance with Medicare and agency policies, and travel locally with a valid NJ driver's license. Evening and night shifts are required.

Qualifications

• Associate Degree in Nursing and New Jersey RN license.
• Hospice or home health experience required.
• Ability to travel locally throughout the North New Jersey territory.
• Strong clinical assessment and documentation skills.
• Ability to work independently in an on-call environment.
• Excellent communication and interpersonal skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess patient needs and provide hospice nursing care with symptom management.
• Coordinate care with attending physicians and interdisciplinary team.
• Document care at point of service per Medicare regulations.
• Administer medications in accordance with regulations and policies.
• Maintain and update patient records and plans of care.
• Communicate significant changes to physicians and supervisor.
• Provide end-of-life counseling and education to patients and families.
• Coordinate equipment, supplies, and services for patients.
