Job summary

Ascend Health is seeking an experienced RN On-Call Nurse to provide compassionate hospice care across Central New Jersey. The role involves rapid assessment, symptom management, care coordination, and responsive support for patients and families during on-call hours.

You will work from an on-call schedule, perform assessments, administer medications per policy, document thoroughly, communicate with physicians and the interdisciplinary team, and travel locally throughout the North New Jersey

Qualifications

• Associate Degree in Nursing required.
• Current and unrestricted New Jersey RN license.
• Previous hospice or home health experience required.
• Strong clinical assessment and documentation skills.
• Ability to work independently and make appropriate clinical decisions in an on-call environment.
• Excellent communication and interpersonal skills.
• Valid driver’s license and reliable transportation.
• Ability to travel locally throughout the assigned North New Jersey territory.

Responsibilities

• Assess patient needs during on-call hours and determine appropriate nursing actions.
• Provide hospice nursing care focused on pain control and symptom management.
• Coordinate care with physicians, hospice team, facilities, and agencies.
• Document accurately and timely at point of service per agency policies and Medicare rules.
• Administer medications per regulations and policies.
• Maintain patient records, plans, outcomes, and changes in condition.
• Communicate with supervisor and interdisciplinary team about patient needs and care plans.
• Coordinate equipment, supplies, and services for patients.
