Job summary
LifeSource Hospice is seeking an experienced On-Call Registered Nurse (RN) to support after-hours hospice care. You will triage calls, perform in-person visits for symptom management, and assist with admissions as needed across Bergen County, NJ.
Travel throughout the region is required, with a focus on compassionate, patient-centered care. The ideal candidate holds an unrestricted NJ RN license, hospice experience, and comfort with EMR and tablets.
Qualifications
- Current, unrestricted New Jersey RN license
- Hospice nursing experience required
- Valid driver's license and ability to travel
- Experience with a PBM (pharmacy benefit manager)
- Comfortable using tablets, iPads, and EMR
- Strong communication and customer-service skills
- Ability to think quickly, prioritize clinical needs, and work independently
- Warm, compassionate, and dependable approach to patient care
Responsibilities
- Triage after-hours calls from patients, families, facilities, and care team
- Make in-person visits for changes in condition or symptom management
- Complete hospice admissions after hours when needed
- Communicate with warmth, professionalism, and sound clinical judgment
- Coordinate medications and PBM needs
- Complete accurate documentation on tablet/iPad
- Travel throughout Bergen County and surrounding areas as needed
