Ascend Hospice & Palliative Care NJ is seeking an RN On Call Nurse for the North Team NJ. The role involves on-call patient visits, hospice care focused on pain and symptom management, and coordinating with physicians and the care team.

The RN must hold a State Certified license, have hospice or home health experience, excellent documentation, and valid driver's license with ability to travel locally within the territory.

Qualifications

• Must hold a State Certified RN license.
• Experience in hospice or home health care required.
• Excellent documentation skills and ability to work independently.
• Strong communication skills.
• Must have valid driver's license and reliable transportation.
• Available to travel locally within assigned territory.

Responsibilities

• Assumes responsibility for a patient visit or on call schedule including the nursing process phases (assessing, planning, implementing, evaluating).
• Provides hospice nursing care focused on pain control and symptom management.
• Coordinates care with attending physicians, hospice staff, and other agencies.
• Maintains up-to-date documentation at point of service and reflects charges per policy and Medicare regulations.
• Administers medications in accordance with laws and Hospice policies.
• Assesses and collaborates with Medical Director to determine patient eligibility for hospice care.
• Regularly re-evaluates patient’s nursing needs and updates the care plan.
• Counsels patient and family on end-of-life needs.
• Communicates changes in patient condition to physicians and IDT members.
