Job summary

Affinity Health Management is seeking an On-Call Hospice Registered Nurse to respond to after-hours patient census and referrals, providing urgent clinical and psychosocial care, pain and symptom management, and coordination with the Hospice Medical Director and on-call administrator.

Responsibilities include after-hours assessments, documenting care, and collaborating with the care team to ensure continuity of care for patients and families in their homes.

Qualifications

• Holds Virginia RN license.
• Hospice, home care, hospital medical-surgical or oncology experience preferred.
• Knowledge of medications and pain management.
• Capable of home visits.
• Ability to consult with physicians and admin staff as needed.
• Maintains patient confidentiality per HIPAA.
• Excellent communication and documentation skills.
• Requires valid driver’s license and insured vehicle.
• Willingness to work after hours and on-call.
• Commitment to the hospice philosophy of care.

Responsibilities

• Available by telephone and email during assigned shifts.
• Conducts urgent physical and psychosocial assessments after hours.
• Maintains a safe environment for the patient.
• Provides nursing care using the nursing process.
• Focus on pain control and symptom management.
• Guides patient and family regarding diagnosis, medications, treatments and progress.
• Documents care timely and accurately.
• Records changes in pain and symptom management.
• Communicates with physicians about patient needs and reports changes.
• Collaborates with IDG and community staff for continuity of care.
• Cooperates with vendors to obtain DME, supplies and meds after hours.
• Coordinates with hospital and community staff to adjust plans of care as needed.
• Conducts after-hours hospice admissions as necessary.
• Participates in mandatory meetings and continuing education.
• Performs other duties as assigned.
