Job summary

Affinity Health Management in Florida seeks an On-Call Hospice RN to respond to patient census and family inquiries after hours, including every other weekend. You will perform urgent clinical and psychosocial interventions, focusing on pain and symptom management and collaborative care with the hospice team.

The role requires independent assessment, timely documentation, and communication with physicians; you will work with the IDG and coordinate with community providers, maintaining HIPAA

Qualifications

• Graduate of an accredited school of nursing.
• Current unencumbered Florida RN license.
• Preferably 2 years hospice/home health or related experience.
• Strong communication, documentation and nursing skills.
• Must have a valid driver's license and reliable vehicle.

Responsibilities

• Responds to patient census and family inquiries after hours.
• Performs urgent physical and psychosocial assessments with appropriate interventions.
• Provides pain and symptom management and maintains patient safety.
• Documents care thoroughly and timely using the nursing process.
• Collaborates with physicians and the IDG for continuity of care.
• Engages in after-hours admissions and required meetings.
