Affinity Hospice Management LLC is seeking an On-Call Hospice RN to respond to patient census and family needs after hours. You will provide urgent clinical and psychosocial care, focusing on pain control and symptom management, and collaborate with the hospice team to maximize outcomes.

Responsibilities include documenting care thoroughly, supporting families, and coordinating with physicians and IDG after hours.

Qualifikationen

• Graduate of an accredited school of nursing.
• Current unencumbered RN license in Virginia.
• Preferably 2 years in hospice, homecare, med-surg or oncology.
• Insured vehicle and valid driver's license.
• Ability to lift and transfer patients and static postures as needed.
• Excellent observation, communication and documentation skills.

Aufgaben

• Responds to patient census and inquiries after hours (nights, weekends, holidays).
• Provides urgent clinical and psychosocial interventions and pain control.
• Documents care thoroughly and timely, with changes/outcomes tracked.
• Collaborates with physicians, IDG and administrators for continuity.
• Coordinates with vendors for DME, supplies and meds after hours.
• Performs after-hours hospice admissions and participates in required meetings.
