Job summary

Affinity Hospice Management LLC is seeking an On-Call Hospice RN to respond to patient census and referrals outside regular hours. You will provide urgent clinical and psychosocial care, manage pain and symptoms, and collaborate with the hospice team to optimize patient outcomes.

Responsibilities include after-hours assessments, documentation, communication with physicians, and coordination with home health and hospital teams to ensure continuity of care for patients and families.

Qualifications

• Graduate of an accredited school of nursing.
• Maintains current unencumbered RN license.
• Commitment to hospice philosophy of care.
• Strong communication with patients, families and team.
• Ability to work independently during after-hours shifts.
• Valid driver’s license and insured vehicle.

Responsibilities

• Responds to after-hours patient census and inquiries by phone or email.
• Performs urgent physical and psychosocial assessments and interventions.
• Provides pain and symptom management and safe nursing care.
• Documents care and updates clinical records promptly.
• Collaborates with physicians, IDG and vendors as needed.
• Participates in required meetings and ongoing education.
