Job summary

Affinity Hospice Management LLC is seeking an On-Call RN to respond to after-hours patient needs, provide urgent clinical care, and coordinate with the hospice team to ensure comfort and symptom control. The role includes after-hours visits, documentation, and collaboration with physicians and family members.

The on-call nurse will support patients at home, communicate with the Medical Director and IDG, and participate in ongoing professional development to maintain high standards of care.

Qualifications

• Graduate of an accredited school of nursing.
• Maintains current unencumbered RN license.
• Preferably hospice/homecare experience.

Responsibilities

• Respond to calls during assigned shifts and provide urgent clinical and psychosocial interventions.
• Assess patients and families after hours and determine appropriate interventions.
• Provide pain control and symptom management and document care thoroughly.
• Collaborate with physicians, IDG and administration on care plans.
• Coordinate with vendors for after-hours supplies and equipment.
• Maintain safe patient environments and complete timely charting.
