Job summary

Beacon Hospice, part of the Optum family, is seeking an Hospice On-Call Registered Nurse for 7 on 7 off shifts. The role provides after-hours clinical support both remotely and in the field, triaging patient needs and coordinating care to ensure continuity and high-quality outcomes.

The nurse will assess patient conditions via phone or in-person visits, collaborate with physicians and care teams, and educate patients and caregivers on symptom management and care plans.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
