Job summary

Optum, part of the Wentworth Hospice and Amedisys family and a member of the UnitedHealth Group portfolio, seeks a Hospice On-Call Registered Nurse. This role provides after-hours clinical support, triage, and coordination of care to ensure high-quality outcomes, with both remote and field-based nursing support.

• Current RN license for the state(s) of assignment.
• Current CPR certification.
• 1+ years of nursing experience.
• Valid driver’s license, reliable transportation, and liability insurance.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
