Job summary

Optimal Hospice seeks a Registered Nurse for a 7on/7off on-call role to care for patients in a hospice setting. The schedule is every other week, Monday-Sunday from 6:30 PM to 8:00 AM, providing comprehensive nursing assessments and care planning.

The RN will administer medications, coordinate care as Case Manager when assigned, and educate patients and families. Colorado licensure, CPR, and hospice experience preferred are required to join our compassionate team.

Qualifications

• Current Colorado RN license.
• Hospice experience preferred.
• CPR certification on active status.
• Excellent communication and assessment skills.

Responsibilities

• Complete initial, comprehensive and ongoing patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the care plan with patient/family involvement as needed.
• Administer medications and treatments as prescribed by the physician.
• Coordinate patient care as Case Manager when assigned by Clinical Supervisor.
• Educate patient and family on self-care and care plan; maintain safe, compassionate care.
