Job summary

The Care Team Hospice is seeking an On-Call Registered Nurse (RN) to provide after-hours and weekend support for hospice patients and families. This 7 On / 7 Off role rotates seven consecutive days off between rotations, enabling extended personal time while contributing to compassionate end-of-life care.

Work from home between visits when not actively responding to patient needs in the field, with an interdisciplinary team supporting you.

Qualifications

• Active and unrestricted RN license in the state of practice
• Minimum of one year nursing experience; hospice/home health/palliative care preferred
• Experience with electronic medical records (HCHB preferred)

Responsibilities

• Respond to after-hours and weekend patient and caregiver needs with nursing assessments and interventions
• Complete scheduled and urgent visits and provide end-of-life care
• Educate patients and families on medications and symptom management
• Coordinate care with physicians, pharmacies, and the interdisciplinary team
• Document accurately in the EMR in a timely manner
