Qualifications

• Graduate of an accredited school of nursing.
• Current unencumbered RN license to practice.
• Two years hospice/homecare/medical-surgical/oncology experience preferred.
• Valid driver’s license with insured vehicle and reliable transportation.
• Ability to work independently and within regulatory guidelines.

Responsibilities

• Respond to hospice census and after-hours referrals and inquiries.
• Conduct urgent physical and psychosocial assessments after hours.
• Provide and document pain control and symptom management.
• Collaborate with physicians and IDG for continuity of care.
• Maintain timely, thorough clinical documentation of patient status.
• Coordinate with vendors for DME, supplies, and meds after hours.
• Communicate changes in patient condition to the bedside team.
