Job summary

Affinity Hospice Management LLC is seeking an On-Call Hospice RN to respond to census and referrals outside office hours, providing urgent clinical and psychosocial interventions and symptom management. You will collaborate with the Hospice team to maximize positive patient outcomes.

The role involves nights, weekends and holidays with comprehensive documentation and coordination with physicians and IDG to ensure continuity of care for patients and families.

Qualifications

• Graduate of an accredited school of nursing.
• Maintains current unencumbered RN license (FL).
• Experience in hospice, homecare or medical-surgical settings preferred.

Responsibilities

• Respond to census and referrals after hours by phone and email.
• Perform urgent physical and psychosocial assessments and determine interventions.
• Provide pain control and symptom management and maintain safe environment.
• Document care thoroughly and timely and communicate with physicians.
• Collaborate with IDG and vendors for after-hours needs and orders.
