Job summary

Affinity Hospice Management LLC is seeking an On-Call Registered Nurse to respond to patient census after hours, including nights, weekends and holidays. The role focuses on urgent clinical assessment, pain and symptom management, and collaboration with the Hospice team to maximize patient outcomes.

The RN will document care thoroughly, communicate with physicians, and provide crisis intervention as needed while maintaining HIPAA compliance and patient confidentiality.

Qualifications

• Graduated from an accredited nursing program and licensed as an RN.
• Ability to perform comprehensive nursing assessments and crisis intervention.
• Familiarity with HIPAA and patient confidentiality requirements.

Responsibilities

• Respond to after-hours census and referrals; provide urgent clinical and psychosocial interventions.
• Perform pain and symptom management; ensure safe patient environments.
• Collaborate with physicians, IDG and community staff; document care thoroughly.
• Maintain timely clinical documentation and communicate changes in patient status to the care team.
• Drive and perform in-home visits; ensure continuity of care across settings.
