Job summary

Affinity Hospice Management LLC is seeking an On-Call Hospice Registered Nurse to respond to patient census and family referrals outside of office hours. You will perform urgent clinical and psychosocial interventions, manage pain and symptoms, and collaborate with the Hospice Medical Director and on-call administrator to maximize patient outcomes.

This role requires independent assessment in crisis situations, thorough documentation, and coordination with hospital and IDG staff.

Qualifications

• Registered nurse with licensure in Florida.
• Two years hospice or homecare experience preferred.
• Knowledge of pain and symptom management.
• Ability to work independently during after-hours.
• Strong documentation and communication.

Responsibilities

• Consistent availability by telephone and email during assigned shifts.
• Conducts urgent physical and psychosocial assessments after hours and determines interventions.
• Provides a safe environment for the patient.
• Uses the nursing process to deliver professional care.
• Focuses on pain control and symptom management.
• Provides guidance to patient and family regarding diagnosis, meds, treatments and progress.
• Documents patient care thoroughly and timely.
• Maintains and submits accurate clinical documentation.
• Communicates with physician regarding patient needs and reports changes in condition.
• Collaborates with IDG for continuity of care.
• Collaborates with vendors for DME, supplies and meds after-hours.
• Coordinates with hospital and community staff to adjust plans of care.
• Conducts after-hours hospice admissions as necessary.
• Participates in mandatory meetings and continuing education.
• Undertakes assigned tasks and other duties as needed.
