Job summary

Innovate Rehab and Wellness is seeking a licensed PRN Certified Occupational Therapy Assistant for a Skilled Nursing Facility in Missouri. The role emphasizes one-on-one therapy to help residents regain independence and improve quality of life, under OT supervision.

You will implement plans, document progress, educate residents and caregivers, and collaborate with nursing, PT, SLP, and other staff. A state license, NBCOT certification, and an OTA degree are required; SNF experience preferred.

Qualifications

• Associate degree from an accredited OTA program.
• NBCOT certification is required.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
