Trilogy Home Healthcare, a Humana company, is seeking a Certified Occupational Therapy Assistant (COTA) to provide high-quality in-home care in St. Lucie County, FL. You will collaborate with Occupational Therapists and the Clinical Services Team to support patients in regaining mobility and independence.

Formación

• Must hold an active Florida COTA license.
• Graduate of an ACOTE-approved OT program.
• Minimum 1+ year of COTA experience; home health preferred.
• Valid Florida driver's license, active vehicle insurance, and clean MVR.

Responsabilidades

• Deliver compassionate, high-quality home health care per physician orders and care plans.
• Collaborate with OTs and Clinical Services Team.
• Assist patients to regain mobility, strength, and independence after illness or surgery.
• Educate patients and families on balance, walking, and fall prevention.
• Assess home safety and mobility to support safe independence.

What You Bring:

• Graduate of an accredited school of Occupational Therapy Program (AOTA-approved)
• Active COTA license in Florida
• Valid Florida drivers license, active vehicle insurance, and clear MVR
• 1+ years of experience as an COTA required, Home Health experience preferred
• Strong initiative and independent judgement
• Alignment with the agency's mission and values
