Job summary

Trilogy Home Healthcare in Florida is seeking an Occupational Therapist (OT) to join our award-winning team for home health visits in St Lucie and Indian River Counties. This full-time role offers a guaranteed salary and the opportunity to impact patients’ mobility and independence in their homes.

You will collaborate with physicians and clinical managers, educate families, and assess safety of home environments to prevent decline.

Qualifications

• Graduate of an accredited OT program (AOTA-approved).
• Active Florida OT license.
• Valid Florida driver’s license with active vehicle insurance and clear MVR.
• 1+ years OT experience; home health experience preferred.
• Strong initiative and independent judgement.
• Alignment with the agency's mission and values.

Responsibilities

• Deliver compassionate, high-quality, personalized care.
• Collaborate with doctors, Clinical Team Managers, and interdisciplinary teams.
• Help patients regain mobility, strength, and independence after illness or surgery.
• Educate patients and families on improving balance, walking, and preventing decline.
• Assess and ensure safe mobility in the home environment.
