Job summary

Trilogy Home Healthcare is seeking a dedicated Occupational Therapist (OT) to join our Flagler County, FL home health team on a PRN/per diem basis. You will deliver high-quality, compassionate care, collaborate with physicians and the clinical team, and help patients regain mobility and independence at home.

Florida-licensed OT with 1+ year experience, valid driver's license, and reliable vehicle required. Flexible scheduling and autonomy offered.

Qualifications

• Active OT license in Florida.
• 1+ year of OT experience in home health preferred.
• Graduate from an AOTA-approved OT program.

Responsibilities

• Deliver compassionate, high-quality, personalized care.
• Collaborate with physicians, Clinical Team Managers, and interdisciplinary teams.

What You Bring:

• Graduate of an accredited school of Occupational Therapy Program (AOTA-approved)

• Active OT license in Florida

• Valid Florida drivers license, active vehicle insurance, and clear MVR

• 1+ years of experience as an OT required, Home Health experience preferred

• Strong initiative and independent judgement
