Job summary

Trilogy Home Healthcare Port Saint Lucie seeks a dedicated Occupational Therapist to provide home health visits in St Lucie and Indian River counties. The OT will collaborate with physicians and the clinical team to improve mobility and independence, educate families, and ensure safe home environments, with flexible scheduling and clinical autonomy.

The role requires an active Florida OT license, 1+ year experience, and a patient‑centered approach.

Qualifications

• Graduate of an accredited OT program (AOTA-approved).
• Active OT license in Florida.
• Valid Florida driver’s license with active vehicle insurance and clear MVR.
• Minimum 1+ year OT experience; home health preferred.
• Demonstrates initiative and independent judgement.
• Aligned with agency mission and values.

Responsibilities

• Deliver compassionate, high‑quality, personalized home health OT care.
• Collaborate with doctors, Clinical Team Managers, and interdisciplinary teams.
• Help patients regain mobility, strength, and independence after illness or surgery.
• Educate patients and families on balance, gait, and fall prevention.
• Assess safety of the home environment and modify as needed.
