Job summary

Trilogy Home Healthcare Port Saint Lucie is seeking a Certified Occupational Therapy Assistant (COTA) to join our Martin County, FL home health team on a PRN/per diem basis. You will deliver high-quality, individualized therapy in patients' homes under physician orders, collaborating with OTs and the clinical services team to support mobility, independence, and safety.

The role values initiative and independence, requires active Florida COTA license, a valid driver's license and insurance, and

Qualifications

• Graduate of an accredited school of Occupational Therapy Program (AOTA-approved).
• Active COTA license in Florida.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• 1+ years of experience as an OT; Home Health experience preferred.
• Strong initiative and independent judgement.
• Alignment with the agency's mission and values.

Responsibilities

• Deliver compassionate, high-quality, and personalized care in accordance with physicians orders and personalized care plans.
• Collaborate with Occupational Therapists and Clinical Services Team.
• Help patients regain mobility, strength, and independence after illness or surgery.
• Educate patients and families on improving balance, walking, and preventing decline.
• Assess and ensure safe mobility in the home environment.
