Job summary

The Occupational Therapist Assistant I (OTA I) for Christ Hospital Home Health Care provides comprehensive skilled care in patients’ homes following the attending physician’s plan of care. The OTA I educates patients and families and ensures patient safety, while maintaining documentation compliant with Medicare/Federal guidelines.

The OTA I collaborates with the supervising occupational therapist and care team to assess needs, guide self-care activities, and help prevent hospital readmissions.

Qualifications

• Minimum of one year of OTA experience in an acute setting is required.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate occupational therapy judgments.
• Detail-oriented and observant.
• Valid driver’s license and auto insurance with your name listed as a driver.

Responsibilities

• Provides skilled occupational therapy services in a patient’s home according to the plan of care.
• Observes and reports patient reactions, changes, and progress.
• Assesses functional problems and works toward goals to improve patient functioning.
• Performs OT procedures and prescribed diagnostics.
• Educates patient and family on treatment procedures and safety measures.
• Communicates with physician, therapist, and coworkers; participates in case conferences.
