Alternate Solutions Health Network in Cleveland, OH seeks an Occupational Therapy Assistant I (OTA I) to provide comprehensive skilled therapy in patients' homes under the guidance of an occupational therapist. You will educate patients and families, ensure safety, and complete documentation in accordance with Medicare and agency protocols.

This role emphasizes accurate assessment, hands-on intervention, and collaboration with physicians, therapists, and caregivers.

Qualifications

• One year of OTA experience in an acute setting required.
• Home care experience is desired.
• Strong communication with patients, families, physicians and coworkers.
• Knowledgeable about therapy best practices.
• Ability to make appropriate occupational therapy judgments.
• Detail-oriented, observant, and timely with tasks.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled OT services in patients' homes per plan of care.
• Observe, record and report patient progress.
• Assess functional problems and support goal achievement.
• Perform OT procedures as prescribed.
• Educate patients and families on treatment and safety.
• Communicate with physician, therapist and coworkers.
• Complete all clinical documentation per agency and Medicare guidelines.
• Maintain active Ohio OTA license.
• Drive to patient locations as scheduled.
• Attend required trainings and on-call duties.
