Job summary

Ashealthnet in Cleveland, OH is seeking an Occupational Therapist Assistant I to provide skilled home-based therapy under the physician's plan of care, educate patients and families, document care in compliance with Medicare guidelines, and coordinate with the care team.

The OTA I will help improve patient independence, perform therapy procedures, monitor progress, and participate in case conferences while maintaining a professional appearance and valid Ohio licensure.

Qualifications

• Minimum of one year of OTA experience, acute setting preferred.
• Home care experience is desirable.
• Ability to effectively communicate with patients, families, physicians and coworkers.
• Knowledge of therapy best practices relevant to occupational therapy.
• Ability to make appropriate occupational therapy judgments.
• Detail-oriented and observant with strong work ethic.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled OT services in a patient’s home according to the plan of care.
• Observe, record, and report patient progress and responses.
• Assess functional problems and implement interventions.
• Guide patient in activities to improve functioning.
• Educate patient and family on treatment procedures and safety.
• Document all aspects of treatment and education in compliance with guidelines.
• Participate in case conferencing and coordinate with care team.
• Attend in-service trainings and mandatory meetings.
