Job summary

Christ Hospital Home Health Care is seeking an Occupational Therapy Assistant I (OTA I) to deliver skilled OT services in patients’ homes in accordance with the physician’s plan of care.

The OTA I educates patients and families, ensures safety, and maintains documentation compliant with Medicare and agency guidelines. The role emphasizes collaboration with physicians and therapists and ongoing professional development.

Qualifications

• Minimum of one year of OTA experience in an acute setting.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Detail-oriented and organized with timely documentation.
• Active OTA license in Ohio.

Responsibilities

• Provide skilled occupational therapy services in a patient’s home per the plan of care.
• Observe, record, report patient changes and progress.
• Assess functional problems and provide interventions toward goals.
• Perform specified OT procedures and diagnostic tests as prescribed.
• Guide patient in使用 therapeutic activities for improving functioning.
• Communicate effectively with physician, therapist and coworkers.
• Participate in case conferencing to coordinate care.
• Educate staff, patient and family on procedures and safety.
• Instruct patient and family on proper use of equipment (wheelchairs, braces, walkers, etc.).
• Help decrease rehospitalizations by teaching preventative measures.
• Foster independence in patients and families to reduce reliance on agency.
• Listen to patient and family members.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Maintain active OTA license in Ohio.
• Communicate changes to scheduler.
• Complete chart audits at agency site.
• Participate in on-call requirements and case conferences.
• Attend in-service trainings and mandatory meetings.
• Participate in orientation/training of new employees.
• Maintain professional appearance as a company representative.
• Drive to patient’s primary location per scheduled visit.
• Document all aspects of subsequent visits within 24 hours.
• Follow plan of care within scope of OT practice.
• Other duties as assigned.
