Job summary

Christ Hospital Home Health Care is seeking an Occupational Therapist Assistant I (OTA) to deliver comprehensive skilled care in patients' homes under physician orders. The OTA I educates patients and families, ensures patient safety, and maintains compliant documentation in line with state and federal regulations.

Responsibilities include providing therapy services, tracking progress, and guiding patients in therapeutic activities to improve functioning.

Qualifications

• At least one year of OTA experience in an acute setting.
• Home care experience is preferred.
• Must communicate clearly with patients, families, physicians and coworkers.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled occupational therapy services in patients' homes per plan of care.
• Observe and document patient progress and reactions.
• Assess functional problems and assist in improving patient functional status.
• Educate patient and family on treatment procedures and safety measures.
• Ensure timely documentation per agency and Medicare guidelines.
• Coordinate with other healthcare professionals to promote care continuity.
