Job summary

Continuum Therapy Partners is seeking an Occupational Therapist for a PRN schedule at Oak View Health & Rehabilitation Center in Orange Park, Florida. The role supports flexible daytime shifts while integrating into a collaborative therapy team focused on client-centered outcomes.

The ideal candidate holds a Master’s or Doctorate in OT, a valid Florida license, and NBCOT certification, with 1 year of OT experience preferred.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
