Job summary

Continuum Therapy Partners, LLC in Orange Park, Florida seeks an Occupational Therapist to join our PRN team. You will enjoy a flexible schedule while remaining part of a supportive rehabilitation team focused on quality care.

Responsibilities include evaluating and treating patients, developing individualized plans, documenting progress, and educating patients and families on adaptive equipment and home safety. Requires OT degree, active license, NBCOT, and 1 year of experience.

Qualifications

• Master's or Doctorate in Occupational Therapy is required.
• Active state OT license and NBCOT certification required.
• 1 year of OT experience preferred.

• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
