Job summary

Continuum Therapy Partners, LLC seeks an Occupational Therapist for Monroe Manor Nursing & Rehab Center in Monroe, Wisconsin. The role requires a Master’s or Doctorate in Occupational Therapy, an active state OT license, and NBCOT certification.

PRN daytime schedule with responsibilities including evaluation, treatment planning, progress documentation, and patient/family education. 1 year of OT experience is preferred.

Qualifications

• Must have state OT license and NBCOT certification.
• 1 year OT experience preferred.
• Proven ability to develop and adjust treatment plans.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
