Job summary

Avondale Health and Rehab is seeking a dedicated Occupational Therapist to plan, organize, develop, and direct OT services within our facility, ensuring the highest quality resident care under current standards and guidelines.

The role supports flexible full-time or PRN scheduling, with a focus on comprehensive patient evaluation, goal-driven treatment plans, and precise documentation. Collaboration with the interdisciplinary team is essential for optimal outcomes.

Qualifications

• Required degree in Occupational Therapy.
• Must hold an Active TN Occupational Therapy license.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy Services in the facility.
• Screen residents to determine the best intervention or treatment for each patient.
• Evaluate residents and develop short- and long-term treatment plans and goals.
• Communicate evaluation findings and plans to residents, families, and the team.
• Schedule resident therapy with other team members and ensure proper documentation and billing.
• Supervise COTA and ensure treatments are documented with appropriate reimbursement practices.

Avondale Health and Rehab is hiring for an Occupational Therapist OT to plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Status: Full-Time with flexible scheduling or Per Diem PRN

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active TN Occupational Therapy license
• Ongoing continuing education and professional development to maintain current licensure and certification
