Job summary

Upperline Health in Kissimmee, FL seeks a Nurse Practitioner to provide clinic-based care with a local physician team. You will develop care plans, diagnose and treat in clinic or home settings, and educate patients on medications, diet, and prevention while coordinating with an interdisciplinary team to optimize outcomes.

You should have a Master’s in nursing, NP board certification, and Florida licensure.

Qualifications

Master’s degree in nursing is required.
Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner is required.
A minimum 6 months Nurse Practitioner experience in primary care/internal medicine.
Value-based care experience preferred.
DEA registration or eligibility is required.
BLS certification is required.
Comfort with home-based patient care.
Experience with EMR systems (AthenaHealth is preferred).

Responsibilities

Develop and implement clinical plans of care for adults with chronic conditions.
Gather history, perform exams, and conduct diagnostics in home or clinic.
Create and adjust treatment plans aligned with patient goals.
Collaborate with interdisciplinary team to address physical and psychosocial needs.
Educate patients and families on medication usage, diet, and crisis prevention.
Prescribe medications as permitted by the Florida Board of Nursing.
Maintain accurate medical records per regulations and policy.
Participate in weekly team meetings and ongoing education.
