Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking an experienced Nurse Practitioner for a part-time, independent role in Illinois skilled nursing facilities. You will manage your own panel, perform primary and episodic care, and build lasting relationships with patients and families.

Qualifications

• Board certification AGNP or FNP is required.

Responsibilities

• Communicate with families and participate in care conferences when appropriate.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner to join our clinical team part time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families. New grads welcome!

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.

Qualifications
