Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) has partnered with a skilled nursing facility to recruit an experienced Nurse Practitioner. You will manage a dedicated patient panel, provide primary and episodic care, and build long-term relationships with residents and families.

New grads are welcome to apply. The role offers a structured 6-hour shift schedule, no on-call or overnight duties, and charting from home.

Qualifications

• Board certification required (AGNP or FNP) active and unrestricted.
• Valid, unrestricted NP license in the state of operation.
• Master’s degree in Nursing from an accredited program.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16-20 patients per day, with a mix of chronic care and acute issues.
• Conduct annual wellness visits and H&Ps, manage chronic conditions.
• Review medications and orders; coordinate with physicians, specialists, and facility staff.
• Stay in touch with families and join care conferences as appropriate.
• Document care efficiently and ensure seamless continuity of care.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity - you'll manage your own panel and build long-term relationships with patients and their families. New grads welcome!

Your Day-to-Day
