This autonomous role focuses on primary and episodic care for skilled nursing facility residents, with your own patient panel and close relationships with families.

The position offers 6-hour shifts, no on-call, charting from home, and productivity-based pay up to $624/day. Full benefits, CME stipend, and a retention incentive are included. New grads are welcome.

Qualifications

• Board certification required (e.g., AGNP, FNP) - active and unrestricted.
• Valid, unrestricted NP license in the state of operation.
• Master's degree in Nursing from an accredited program.
• Skilled nursing or long-term care experience strongly preferred.
• Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.

Responsibilities

• See roughly 16-20 patients daily, balancing long-term care with acute issues.
• Manage wellness visits, H&Ps, and chronic disease care.
• Review medications, orders, and coordinate with physicians and facilities.
• Engage with families and participate in care conferences as appropriate.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner to join our clinical team full time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity - you'll manage your own panel and build long-term relationships with patients and their families. New grads welcome!

Your Day-to-Day

You'll see roughly 16-20 patients a day - a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.
