Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking an experienced, autonomous Nurse Practitioner to join our clinical team part time, providing primary and episodic care to residents in skilled nursing facilities. New grads welcome; you’ll manage your own panel and build lasting relationships with patients and families.

You’ll see roughly 16–20 patients daily, with 6-hour morning rounds. Chart from home; no on-call or overnights.

Qualifications

• Board certification (e.g., AGNP, FNP) active and unrestricted.
• Unrestricted NP license in the state of operation.
• Master's degree in Nursing from an accredited program.
• Skilled nursing or long-term care experience preferred.

Responsibilities

• See roughly 16–20 patients per day, mixing long-term chronic care with acute issues.
• Conduct annual wellness visits and H&Ps, manage chronic conditions and medications.
• Coordinate with physicians, specialists, and facility staff to ensure seamless care.

About the Role

We're looking for an experienced, autonomous Nurse Practitioner to join our clinical team part time, providing primary and episodic care to residents in skilled nursing facilities. This is an independent role built around continuity — you'll manage your own panel and build long-term relationships with patients and their families. New grads welcome!

Your Day-to-Day

You'll see roughly 16–20 patients a day — a mix of long-term chronic care and acute issues. That means annual wellness visits and H&Ps, managing chronic and co-morbid conditions, responding to acute changes, reviewing meds and orders, and coordinating with collaborating physicians, specialists, and facility staff to keep care seamless. You'll also stay in touch with families and join care conferences where it makes sense.
