SourcePro Search, LLC is seeking a full-time Nurse Practitioner to provide comprehensive primary care within a community health center in the Greater Sacramento area, CA. The role supports diverse, underserved populations with a collaborative care team and physicians.

Compensation ranges from $131,000 to $170,000 annually, with relocation assistance available. Schedule includes weekdays from 8-5 or a 4-day 10-hour shift option.

Competenze

• Active California RN and Nurse Practitioner licenses required.
• Current DEA certification.
• Current BLS certification.
• 1-2 years of clinical experience in community health or underserved populations preferred.
• Ability to treat three patients per hour.
• Proficient with EPIC EMR.

Mansioni

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and therapeutic interventions within scope of practice.
• Develop and implement treatment plans in collaboration with the Medical Director.
• Monitor patient progress and modify care plans as appropriate.
• Initiate emergency procedures when clinically indicated.
• Provide preventive care, health education, and counseling services.
• Maintain accurate and timely documentation in accordance with agency and regulatory standards.
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care.
