Clear Concepts, Inc. seeks a full-time Nurse Practitioner to provide comprehensive primary care in a community health setting in the Greater Sacramento area. Relocation assistance is available and compensation ranges from $131,000 to $170,000 annually based on experience.

Responsibilities include evaluating patients, diagnosing conditions, ordering tests, prescribing therapies, and collaborating with physicians. Great for those committed to underserved populations with EPIC experience.

Qualifications

• Active California RN and Nurse Practitioner license.
• Current Nurse Practitioner board certification.
• Current DEA certification.
• Current BLS certification.
• 1-2 years clinical experience, preferably in community health.
• Commitment to serving underserved communities.
• Comfortable treating three patients per hour.
• EPIC experience.
• In-basket management competency.
• Bilingual English and Russian/Spanish preferred.

Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and diagnostics.
• Prescribe medications and interventions within scope of practice.
• Develop and implement treatment plans with Medical Director.
• Monitor progress and modify care plans as needed.
• Initiate emergency procedures when indicated.
• Provide preventive care, health education, and counseling.
• Maintain accurate documentation per standards.
• Collaborate with physicians and care teams for coordinated care.
