Job summary

Eden Capital Careers seeks a full-time Nurse Practitioner to provide primary care at a community health center in the Greater Sacramento area. The role includes collaboration with physicians and care teams to serve diverse, underserved populations.

Responsibilities include comprehensive evaluations, diagnosing conditions, ordering tests, prescribing treatments, and developing collaborative care plans. Relocation assistance is available.

Qualifications

• Active California RN and Nurse Practitioner license.
• Current Nurse Practitioner board certification.
• Current DEA certification.
• Current BLS certification.
• 1–2 years of clinical experience preferred, community health focus ideal.
• Bilingual English and Russian/Spanish/Farsi preferred.

Responsibilities

• Perform comprehensive patient evaluations, history and physical exams.
• Diagnose and manage acute and chronic conditions.
• Order and interpret labs, imaging, and other diagnostics.
• Prescribe medications and interventions within scope.
• Develop and implement treatment plans with Medical Director.
• Monitor progress and adjust care plans as needed.
• Initiate emergency procedures when indicated.
• Provide preventive care, health education and counseling.
• Maintain accurate documentation per standards.
• Collaborate with physicians and care teams.
