Essenti Talent is seeking a full-time Nurse Practitioner to join our community health team serving the Greater Sacramento area. The role delivers comprehensive primary care in a collaborative setting with physicians and multidisciplinary staff.

Responsibilities include patient evaluations, management of acute and chronic conditions, lab orders, prescribing within scope, and care planning. Relocation assistance is available, and experience with EPIC is a plus.

Qualifikationen

• Active California RN and Nurse Practitioner license.
• Current Nurse Practitioner board certification.
• Current DEA certification.
• Current BLS certification.
• 1-2 years of clinical experience preferred, community health or underserved populations.
• Comfortable treating three patients per hour.
• EPIC experience.

Aufgaben

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and therapeutic interventions within scope of practice.
• Develop and implement treatment plans with Medical Director.
• Monitor patient progress and modify care plans as appropriate.
• Initiate emergency procedures when clinically indicated.
• Provide preventive care and health education.
• Maintain accurate documentation per standards.
• Collaborate with physicians and other healthcare professionals.
