Job summary

Qualifications

• 2+ years nurse practitioner experience in Primary Care/Internal Medicine.

Responsibilities

• Develop and implement clinical plans of care for adult patients facing chronic conditions.
• Gather patient history, physical exam, and diagnostics within a clinic setting.
• Collaborate with the interdisciplinary team to meet patient and family needs.
