6 Degrees Search seeks a full-time Advanced Practice Clinician (NP/PA) to deliver comprehensive primary care in a community health center for the Greater Sacramento area. You will work with physicians and care teams to serve diverse, underserved populations in a collaborative setting.

Requirements include an Active California PA license, NCCPA certification, DEA, and BLS. 1–2 years of clinical experience is preferred; bilingual candidates are strongly preferred.

Qualifikationen

• Active California Physician Assistant license.
• NCCPA board certification.
• Current DEA certification.
• Current BLS certification.
• 1–2 years of clinical experience preferred, especially in community health.
• Bilingual in English and Russian, Farsi, Pashto/Pushto, or Spanish preferred.

Aufgaben

• Perform comprehensive patient evaluations, including history and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Order and interpret laboratory tests, imaging, and other diagnostics
• Prescribe medications and therapeutic interventions within scope of practice
• Develop and implement treatment plans in collaboration with the Medical Director
• Monitor patient progress and modify care plans as appropriate
• Initiate emergency procedures when clinically indicated
• Provide preventive care, health education, and counseling services
• Maintain accurate and timely documentation in accordance with agency and regulatory standards
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care
