Job summary

ComplexCare Solutions (CCS) is seeking a Nurse Practitioner for per diem home health and telehealth assessments across Douglas County, OR. The NP will conduct comprehensive member assessments, document findings, and coordinate with PCPs and health plans to support care in the home.

Compensation includes a sign-on bonus of $3,500, in-home visit rates, telehealth options, and a path to a 401K with automatic 3% contributions. Strong CMS/HEDIS knowledge and EHR proficiency are essential for success.

Qualifications

Active nursing license in good standing and NP certification.
Knowledge of CMS Regulations and NCQA HEDIS Guidelines.
Experience documenting and coding per CMS/DHHS; familiar with ICD-10/CPT-4.
Ability to work autonomously in a remote, home-based setting.
Proficient with EHRs and secure messaging; strong communication with PCPs and health plans.

Responsibilities

Complete comprehensive reviews of assigned member population and submit encounter documentation.
Document all active medical conditions per guidelines and client requirements.
Communicate assessment results and recommendations to PCPs and health plans; coordinate follow-ups.
Suggest information to improve member knowledge on chronic conditions and testing.
Address gaps in care and escalate emergent situations per policy.
Perform, document, and communicate results of Point of Care testing.
Maintain HIPAA compliance and company policies; adhere to data privacy.
Maintain ~37 hours per month availability (about 25 slots).
Support autonomy in remote clinical environment with sound clinical judgment.
